Request an appointment using our secure online form. Our goal is to contact you by phone by next business day to review your medical and financial information.
You can also call our office at (281) 664-2202 during normal business hours, 7 a.m. to 7 p.m., Monday through Thursday or 7 a.m. to 6 p.m., Friday to schedule
Patient Forms:
Pleaseprint forms and eliminate some of the paperwork you would normally fill out in the office before seeing the therapist. If you forget to bring the forms with you, you will have to fill them out in the office before the therapist can see you. ( Please print and sign Medical History, Informed Consent and Financial Policy)

Patients and others can learn about health-related topics by viewing multimedia patient education material, videos prepared by Advanced Orthopaedics and Sports Medicine surgeons and therapists or material from Orthopaedics connection website of Academy of OrthopaedicSurgeons.Patients can also prepare for pre and postop instructions to common surgeries.

The mission of this program is to offer free educational seminars and workshops in the community that we serve.The physicians are dedicated to serving you and our local community and can be seen at the sidelines of many of Houston's high school football games.
